MyDDS LAB Customer Loyalty Rewards Program: Terms and Conditions
By participating in the MyDDS LAB Customer Loyalty Rewards Program (“the Program”), you (“Customer”) agree to the following Terms and Conditions, as may be revised or updated from time to time. Please read these carefully before participating.
1. Program Overview
The MyDDS LAB Customer Loyalty Rewards Program allows Dental Office Customers to earn loyalty points (“Points”) for every dollar spent on eligible purchases from DDS Lab. Points can be redeemed for customized marketing materials, services or supplies offered by DDS Lab.
2. Earning Points
- Customers will earn one Point for every $1.00 spent on eligible purchases at DDS Lab.
- Eligible purchases include all standard transactions made through DDS Lab, excluding taxes, shipping fees, and any other non qualifying charges.
- Points are awarded only when payments are fully processed and cleared.
3. Expiration of Points
- Points expire one (1) year from the date of the transaction in which the points were earned or at the end of each calendar year (whichever comes first).
- Expired Points will be forfeited and cannot be reinstated.
4. Redeeming Points
- Points can be redeemed for customized marketing materials, services or supplies available through the DDS Lab rewards catalog.
- The exact rewards available, along with the number of Points required for each item, are subject to change at DDS Lab’s discretion.
- Points and Rewards are non transferable and cannot be exchanged for cash or credit.
- Points cannot be combined with other discounts or promotions unless expressly permitted by DDS Lab.
5. Account Maintenance
- The Program is linked to the Customer’s MyDDS LAB Portal account, which must be in good standing for Points to be earned or redeemed (e.g., all amounts due and owing to DDS Lab have been paid in full).
- DDS Lab reserves the right to review and, at its discretion, disqualify any account that appears to be engaging in fraudulent or abusive activity, including but not limited to multiple accounts or manipulation of the Program.
6. Changes to the Program
- DDS Lab reserves the right to modify, suspend, or terminate the Program at any time, with or without notice.
- DDS Lab interpretation of the terms and conditions of the Program will be binding upon the Customer absent manifest error.
- If DDS Lab terminates the Program, any unredeemed Points at the time of termination will be forfeited and cannot be redeemed or refunded.
7. Limitations and Exclusions
- DDS Lab may restrict the eligibility for Points or redemption for certain products, services, or promotions.
- The Program is void where prohibited by law.
- DDS Lab is not responsible for lost, stolen, or unauthorized use of Points or rewards.
8. Privacy
- By participating in the Program, you agree to the collection, use, and disclosure of your personal information in accordance with DDS Lab’s Privacy Policy.
- Personal information may be used to send promotional offers, updates, or communications related to the Program.
9. Limited Warranties, Limitation of Liability, and Disclaimers
- For any goods redeemable with Points that are not manufactured by DDS Lab (e.g., VITA classical shade guide), DDS Lab makes no representations or warranties whatsoever and such goods and services are subject only to the warranty, if any, offered by such third-party manufacturer. For any goods and services redeemable with Points that are performed or manufactured by DDS Lab, such goods and services are subject to the Limited Warranty on the DDS Lab website.
- Customer’s participation in the Program is subject to all applicable Terms and Conditions on the DDS Lab Website including, but not limited to, its Limitation of Liability policy and its Disclaimers. Any inconsistency between the Program Terms and Conditions and the Terms and Conditions on the DDS Lab Website shall be resolved in favor of the Program Terms and Conditions.
10. Miscellaneous
- The redemption of Points for certain goods and services (e.g., intraoral scanner placement) is subject to the Customer’s execution of a separate agreement governing the rights and obligations of Customer and DDS Lab with regard to such scanner placement. Any inconsistency between the Program Terms and Conditions and any term or condition in such separate agreement executed by Customer and DDS Lab shall be resolved in favor of the separate, executed agreement.
- These Terms and Conditions constitute the entire agreement between you and DDS Lab regarding the Program and supersede any prior agreements or understandings related to the Program.
- Any disputes arising from the Program will be governed by the laws of Florida, without regard to its conflict of law principles.
- If any part of these Terms and Conditions is deemed invalid or unenforceable, the remainder will remain in full force and effect.
By participating in the MyDDS LAB Customer Loyalty Rewards Program, you acknowledge that you have read, understood, and agreed to these Terms and Conditions.
